    Beyond Infrastructure: Broadband for Development in Remote and Indigenous Regions

    Recent telecommunications stimulus projects in the U.S. and Canada were intended to increase availability of broadband through funding infrastructure investments, largely in rural and remote regions. However, true access involves more than availability; it also includes affordability and adoption. This paper presents a framework for analyzing broadband adoption that takes into consideration geographical, economic and cultural environments in indigenous communities. It includes an overview of potential social and economic impacts of broadband in remote areas, using examples from the Alaska study and the Canadian North. It then reports on results of an evaluation of Internet use and potential adoption of broadband in remote indigenous communities of southwest Alaska. Finally, the paper provides a comparative analysis of U.S. and Canadian policies intended to achieve affordable access to broadband for rural users and sustainable business models for rural broadband providers.Ye

    Digital Diversity: Broadband and Indigenous Populations in Alaska

Sponsored by the Ford Foundation.Alaska Natives comprise several cultural and linguistic groups including Inupiat, Yupik, Athabascan, Aleut, Tlingit and Haida, organized into some 226 tribes. Approximately two-thirds of the indigenous population live in more than 200 rural villages, most of which are remote settlements with fewer than 200 people and no road access. Since the late 1970’s, all communities with at least 25 permanent residents have had telephone service, but broadband connectivity remains limited. The major mechanism for extending Internet access to rural Alaska has been federal universal service funds, specifically the E-rate program that subsidizes Internet access for schools and libraries, and the Rural Health program that subsidizes connectivity for rural health clinics and hospitals. Under the federal Stimulus program, Alaska has also recently received funding for infrastructure to extend broadband in southwest Alaska, for improved connectivity for rural libraries, and for training and support for rural public computer centers. These initiatives primarily support improvements in Internet and broadband availability for rural Alaska. However, this paper proposes a more rigorous framework including not only availability, but more broadly access, and also adoption, and examines how these concepts apply to Alaska natives. The paper also examines other elements of digital diversity, including innovation in applications and content, ICT entrepreneurship, and participation in telecommunications policy-making

    After Broadband: A Study of Organizational Use of Broadband in Southwest Alaska

    The purpose of this research was to gain a preliminary understanding of how organizations including large and small businesses, Native corporations and organizations, and local and regional governments are using broadband that is now available in much of southwest Alaska. To learn about community access to broadband, interviews were also conducted with library and school staff in communities where broadband had been installed under the OWL (Online with Libraries) program. Further, the study identifies research from other sources that could help to predict what socio-economic impacts the availability and adoption of broadband may have in rural Alaska. Financial institutions use online connections for teller services and credit and debit card processing, and stated that more people in rural communities now have debit cards that they can use for online purchases and bill paying. Large retailers use online services for payroll, for pointof-sale (POS) transactions, and online ordering. Seafood processors rely heavily on connectivity with their head offices (generally in the lower 48) for administrative services including payroll, accounting, shipping and receiving, purchasing, and ERP (enterprise resource planning), and access data base software to track fish tickets. Seafood processors also provide Internet access for their employees, most of whom are seasonal and from other states or countries. Tourism businesses use broadband for online reservation systems and for guests, who increasingly demand connectivity even for remote vacations. Village corporations and tribal councils use online services to help their residents obtain hunting and fishing licenses and fishing permits, to learn about funding opportunities, and to file reports on grants. Local Governments connect online for interoffice communications and for payroll and other administrative functions. Other online applications and services include providing remote desktop access from other agency sites, use of online tools for land management and mapping, training including webinars for workforce development, and providing access to social services for clients. An economic development organization sends newsletters to communities electronically and packets of documents to its board members rather than relying on fax or courier. Websites are important for tourism-related businesses to advertise and promote their businesses and for nonprofits and local governments to provide information about their services. 5 Broadband now plays many roles in rural education. Most students are required to use the Internet for class assignments. High school students can connect to classes in advanced subjects in other communities, and may complete online courses for college credit. Libraries remain important locations for community access, with residents going online to connect with friends on Facebook, as well as to download content for e-books, file income tax, and apply for jobs and government benefits. School and library Wi-Fi provides access inside and near the buildings for residents with smartphones. Despite enthusiasm for broadband and the adoption of many broadband-based applications and services, most organizations interviewed identified problems with broadband, particularly with the pricing, stating that the terrestrial broadband network is too costly for them to take full advantage of online services and applications. While the scope of this study was too limited to estimate long-term benefits, it found that broadband is highly valued and increasingly important to businesses and nonprofit organizations and local governments in southwest Alaska. Broadband helps businesses to be more efficient in their operations and to extend their reach to new customers and suppliers. It also helps to improve the effectiveness of public sector services such as those provided by borough and city governments and extends access to education and training.     Ultrasound Guided Placement of Single-Lumen Peripheral Intravenous Catheters in the Internal Jugular Vein

    Introduction: The peripheral internal jugular (IJ), also called the “easy IJ,” is an alternative to peripheral venous access reserved for patients with difficult intravenous (IV) access. The procedure involves placing a single-lumen catheter in the IJ vein under ultrasound (US) guidance. As this technique is relatively new, the details regarding the ease of the procedure, how exactly it should be performed, and the safety of the procedure are uncertain. Our primary objective was to determine the success rate for peripheral IJ placement. Secondarily, we evaluated the time needed to complete the procedure and assessed for complications. Methods: This was a prospective, single-center study of US-guided peripheral IJ placement using a 2.5-inch, 18-gauge catheter on a convenience sample of patients with at least two unsuccessful attempts at peripheral IV placement by nursing staff. Peripheral IJ lines were placed by emergency medicine (EM) attending physicians and EM residents who had completed at least five IJ central lines. All physicians who placed lines for the study watched a 15-minute lecture about peripheral IJ technique. A research assistant monitored each line to assess for complications until the patient was discharged. Results: We successfully placed a peripheral IJ in 34 of 35 enrolled patients (97.1%). The median number of attempts required for successful cannulation was one (interquartile range (IQR): 1 to 2). The median time to successful line placement was 3 minutes and 6 seconds (IQR: 59 seconds to 4 minutes and 14 seconds). Two lines failed after placement, and one of the 34 successfully placed peripheral IJ lines (2.9%) had a complication – a local hematoma. There were, however, no arterial punctures or pneumothoraces. Although only eight of 34 lines were placed using sterile attire, there were no line infections. Conclusion: Our research adds to the growing body of evidence supporting US-guided peripheral internal jugular access as a safe and convenient procedure alternative for patients who have difficult IV access

    Investing in Infrastructure in Developing Regions: Innovative Strategies and Policies

    Recent innovations in technology and policy are significantly increasing access to basic telecommunications services in the developing world. For example, in many developing countries, there are now more wireless lines than fixed lines; for many new subscribers, their mobile phone is their first and only phone. This dramatic growth in connectivity is due partly to the advantages of wireless technologies in fast deployment of new networks, but perhaps more to the innovations in regulation and policy that have fostered investment. Where countries have introduced competition in wireless/mobile services, rates have come down, and wireless carriers have introduced features such as rechargeable cards and inexpensive text messaging. The result has been dramatic growth in access to basic communications. However, access to the Internet is still very limited in most developing regions, and broadband is almost nonexistent. Where access is available, the price is often beyond the means of most potential users and bandwidth is extremely limited, severely hampering exploitation of the Internet’s potential for social and economic applications – in commerce, education, health, and other services. This paper will examine how technological and regulatory innovations might increase Internet access and bridge the broadband divide. For example, new technologies such as Wi-Fi are being used in industrialized countries to establish "hot spots" for high-speed access in businesses and cyber cafés. This approach may be more attractive in terms of cost and fast deployment than wireline or cable. Fixed wireless may also be used to extend last mile access. Small satellite terminals (VSATs) may be the least cost solution in isolated and remote areas. Yet access to the backbone network may still be expensive unless regulatory policies that allow or even foster competition in the national network. At the local level, competition is needed so that users can choose an alternative to the incumbent network such as a VSAT or wireless network. The paper will also examine how targeted subsidies may be used to increase broadband access. A policy of providing discounts or other funding to end users may be more effective than the more traditional policy of subsidizing the carrier of last resort. This model has been successfully used in the U.S., where schools, libraries, and rural health centers are empowered through universal service subsidies to solicit bids for services from carriers. The paper will conclude with a summary of lessons learned from these innovations in technology and policy that could help to accelerate access to affordable and reliable Internet access in developing regions
